Management of patients with non-valvular atrial fibrillation (AF)and an acute coronary syndrome (ACS), either STEMI (ST-elevation myocardial infarction) or a NSTE-ACS (non-ST-segment elevation ACS, including unstable angina and myocardial infarction), may be particularly challenging. Most AF patients would be taking oral anticoagulant therapy for stroke prevention,1 and various antithrombotic drugs have to be added to facilitate the restoration of coronary flow and to reduce the risk of recurrent ischaemic events, such as stent thrombosis.2,3 Such complex treatments inevitably increase risk of bleeding and must be carefully balanced.1–5 A multiplicity of choices, including new orala nticoagulants (NOACs) and antiplatelet drugs, may add to the complexity of clinical decision-making.
